First, find the slope of the line segment connecting the 2 given points:
        10-(-2)         12
m = ----------- = --------- = 3
         3-(-1)            4

Use the point-slope form:    y-(-2) = 3(x-[-1]), or y = 2 + 3x - 3, or y = 3x - 1 

Can you now fill in the blanks properly?
